How do I get the new DIS (Delivery instruction booklet)?

If you have used complete DIS Booklet and want it to be re issued, than you have to fill the Requisition Slip (pink slip) which is attached in the end of your old DIS Booklet.

However, In case the DIS is lost then you have to submit the "DP Lost Booklet Form" in your Branch or you can also send the same to our Noida Office address i.e.

Master Capital Services Ltd.

C-56, Sector -63, Noida – 201301 (Uttar Pradesh).

Please note that the new DIS shall be sent on Communication Address only.

Alternatively, you can also collect the DIS Booklet by visiting our offices in person located in Ludhiana, Chandigarh, New Delhi or Noida. The Client must show his/ her identity for the same.
